Geriatric NP — Community-Based Primary Care

Matter Health is seeking an Advanced Practice Provider/Nurse Practitioner to support older adults in Atlanta, GA. In this fully community-based position you will manage a dedicated panel of older adults as their ongoing primary care provider, building strong, lasting relationships within the communities you serve. APPs work with a high degree of clinical autonomy while collaborating closely with an interdisciplinary care team and a dedicated Medical Director for support in complex cases. #J-18808-Ljbffr Matter Health
