Nurse Practitioner

Nurse PractitionerContract Length: 6+ months Location: Mishawaka, IN 46544 Shift: FT, 30-35 hours weekly, every other weekend. Hours for clinic are Mon-Fri 8a-7p (1 hour lunch) Sat 9a-530p (30min lunch) Sun 9a-430p (30min lunch) Candidates must have prior primary care or urgent care experience. Do not submit candidates who do not meet this requirement.Primary ResponsibilitiesClient Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients.The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients.This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.This role will report to the practice manager.Include screenshot of BLS and license. (BLS from ARC or AHA is required.)Experience:A MINIMUM of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine, primary care practice, or urgent care required.Working knowledge and understanding of quality measurement in the management of chronic disease conditionsSelf-motivated, prioritizes and solves problems, takes initiative, and advocates for their patients and their practiceEducation:Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role.Candidates must have prior primary care or urgent care experience.
