Job Summary:
A Home Health / Hospice Registered Nurse (RN) position is available in Beaver Dam, Wisconsin. This role involves providing comprehensive home health and hospice services to a primarily geriatric population, with occasional pediatric cases. Candidates are required to bring their license on the first day of assignment and must have at least one year of relevant experience. The position offers a Monday through Friday schedule with daily hours from 8:00 AM to 4:30 PM, and candidates should be prepared to perform routine and OASIS visits.

Job Details:

• Location: Beaver Dam, Wisconsin
• Certifications Required: Basic Life Support (AHA)
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM (end time may vary), minimum of 40 hours per week
• Required Qualifications: BLS (AHA); Minimum of 1 year (experience); valid license; willingness to work in home health and hospice settings with travel experience; ability to perform complex wound care, IV administration, ostomy care, central line care, NPWT, and patient/family education; critical thinking abilities; empathy and compassion for end-of-life care
• Additional Information: Clocks in and out at the facility; no floating required but will do both home health and hospice visits; no vents used but Cpap & BiPap on occasion, patients self-manage; manager available after hours/weekends/holidays; must note if previously worked for the organization with an end date at least 6 months prior to submission; one day of computer-based orientation plus 2-5 days of field training depending on experience; no float rotation; scrubs should be solid color with closed-toe, non-slip shoes.
