Key Responsibilities\n

• Conduct comprehensive assessments to meet with clients and discuss their individual needs, preferences, and treatment goals before crafting tailored rehabilitation plans.

• Design and implement customized rehabilitation plans that reflect clients’ educational backgrounds, aptitudes, physical abilities, and aspirations, regularly updating these plans as care progresses.

• Collaborate closely with an interdisciplinary team of medical professionals—including physicians, occupational therapists, and psychologists—to develop and execute holistic client rehabilitation programs that address specific mental health concerns.

• Maintain thorough case files and documentation for each client, ensuring accurate records of personal information, eligibility, insurance coverage, and services provided.
\n
• Closely monitor client progress through regular check-ins and documentation, adjusting goals and strategies as necessary to ensure successful outcomes.

• Provide consistent support and guidance during clients’ participation in job training, occupational placements, and related programs, actively resolving any challenges that may arise.

• Engage with clients’ families and friends to facilitate effective communication and address needs for broader support.

• Timely reporting and analysis of client needs to ensure that all documentation is up-to-date and accurate.

• Make informed recommendations and referrals to additional resources, enhancing the care experience.

Qualifications\n
