Physical Therapist (PT) - Home Health

Skilled Home Health Therapist
Day shift with block scheduling. 1–2 weekend shifts per contract; 6–8 skilled home visits per day (minimum of 6 points per day).
Home health setting with an interdisciplinary support team.
Minimum of 1 year of home health experience required. OASIS training preferred. Current state license (Florida license or compact license) required at the time of submission. BLS certification required. Valid driver's license and reliable vehicle for home visits. Experience with home health documentation; OASIS experience preferred.
Conduct assessments and provide cardiopulmonary care. Deliver orthopedics and neurological care, including gait training and assistive device assessments. Implement home exercise programs and ADL training. Provide trach care and care for prosthetics/amputations. Educate patients and families on safety/injury prevention and medication understanding. Document care in electronic health records as per facility guidelines.
COVID-19 vaccine required (medical/religious exemptions allowed with form). Influenza vaccine or declination required; mask must be worn while at work. Candidates must reside at least 100 miles from the facility.
