We re hiring a Home Health Physical Therapist (PT) who wants strong earning potential, autonomy in the field, and the opportunity to provide personalized care in a smaller, high-touch environment.

You ll step into a home health role focused on evaluations, treatment, functional improvement, and helping patients regain mobility and independence in their own homes. You ll manage your territory with considerable autonomy while joining an established organization with an existing patient base throughout Northern Connecticut.

What makes you a great fit

You are a licensed Physical Therapist in Connecticut or eligible for licensure
You bring professional Physical Therapy experience
Home health experience is strongly preferred
You re comfortable working independently in the field
You enjoy helping patients improve mobility, function, and independence at home
You value autonomy and a smaller, high-touch organization

Compensation & Benefits

Base salary: $105,000 $130,000
Additional productivity incentives available, with final structure being established
