If this sounds like you, let's talk!El Paso, TexasMajor ResponsibilitiesAs the SNF-ist, you will act as a Skilled Nursing Facility's leading provider of internal medicine services on Saturday and Sunday.You will collaborate with the facility's interdisciplinary team and physicians to reduce hospitalizations on weekends.You will work with the entire facility's patient population - short and long-term residents with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians.Evaluation of new-admission visitsCoordination of discharge visitsEvaluations for patients exhibiting changes in conditionAs a SNF-ist, you will provide types of care, including but not limited to:Advanced Care PlanningProvide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.Prescribe all necessary medications and treatmentsPerform comprehensive physical examinations of patientsDevelop and implement patient management policies and proceduresFacilitate referrals to other healthcare professionals and medical facilitiesCommunicate with attending physicians and/or specialists regarding patient careBenefitsA healthy work-life balance with flexibility.Malpractice insurance and tale coverage for $1MM/$3MM coverage will be provided by Company.Revenue-split compensation.401k with Company matchQualificationsAdvanced Practice Licensure (APN) with a Texas licenseMust have an active license and DEA number.Previous experience in nursing or other medical fields.Ability to build rapport with patients and their loved ones.Ability to thrive in a fast-paced environment.Excellent written and verbal communication skills.Strong leadership experience with emphasis on teamwork in a healthcare setting.Master's degree required
