Maintain patient confidentiality in accordance with Phoenix House policies and procedures. Oversee unlicensed assistive personnel (e.g., CNAs, Medical assistants, and Medical Technicians). Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub‑Q, intradermal) in accordance with NYS scope of practice and facility policies. Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership. Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership. Engage in health education and reinforce treatment plans, medication adherence, and self-care strategies. Other duties as assigned. Required Education High School Diploma or GED Graduate of an accredited nursing program Licenses/Certifications Current NYS Licensed Practical Nurse First Aid Training CPR Training Narcan Training Infection Control Certification Child Abuse Certification Required Experience At least three years of related experience Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and/or drugs.
