About This Role The Case Manager RN position at CVS Health is a fully remote role focused on telephonic assessment, planning, and coordination of case management activities for members of Federal Plans. This role is ideal for experienced nurses looking to enhance member wellness through individualized care. Highlights Pay: $54,095 - $129,615/year Location: Remote, AZ Schedule: Full-time, Monday through Friday, 8 hour shift between 8am to 5:30pm Arizona time Residency: Must live in either PST, MST, or Arizona Time zones What You'll Do Assess, plan, implement, and coordinate case management activities telephonically. Evaluate medical needs of members to promote overall wellness. Develop proactive courses of action to enhance member outcomes. Apply data-driven methods to identify members for individualized case management. Conduct comprehensive clinical assessments. Advocate for patients within existing healthcare coverage. Promote quality, cost-effective outcomes and suggest improvements. Identify and escalation quality of care issues through established channels. Utilize assessment techniques to determine member's health literacy and readiness to change. Employ motivational interviewing skills to engage members and promote lifestyle changes.
