Job Details

Leadership role overseeing 3 full-time case managers, 1 assistant, and 2 per diem staff
Team includes RNs and Social Workers

Requirements
RN with BSN or Social Worker with MSW or Social Work License
CCM or ACM certification required within first year of hire
Strong leadership and team management skills
Experience in case management preferred
