Responsibilities Engaging children and families involved with multiple systems. Developing and facilitating family‑centered teams. Conducting strengths and needs assessments. Developing and implementing individualized Wraparound plans. Collaborating with community partners. Modeling appropriate social skills for parents and their children. Communicating effectively with county workers. Supporting parents to improve life skills to be able to manage new needs as they arise. Participating in training, credentialing, and evaluation processes for High Fidelity Wraparound. Participating in monthly group coaching meetings and case consultations. Participating in other trainings as applicable. Maintaining client files to include assessments, program plans, client progress, and assessments by other professionals, intake forms, client outcome data, referrals, and other pertinent information. What You Bring You have a bachelor’s degree in social work, psychology, parent education, criminal justice, or a related field, or you can demonstrate equivalent knowledge gained through alternative courses of study and life experience. You have knowledge and experience working with culturally and racially diverse populations in lower socio‑economic conditions. You can speak multiple languages, which is highly valued. You demonstrate strong organizational skills, allowing you to manage multiple priorities and meet deadlines. You understand the importance of customer service and professionalism. You showcase skills in conflict management. You are computer literate, with proficiency in MS Office. You can work both independently and as part of a team. You use good judgment in recognizing the scope of your authority. You can travel to client sites within a 15‑mile radius of the office. You have a valid driver’s license, access to reliable transportation, and the ability to meet FamilyWise’s driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit (not applicable for new hires living out of state).
