LICENSED PRACTICAL NURSE

Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment./pulliProvides medication management within the residential setting/liliProvides medication administration to clients in the residential environment in accordance with authorized prescriptions/liliDocuments the medication administration process in each clients Medication Administration Record/liliProcures and logs in medications received for each client within the facility/liliTracks client compliance with required medications, identifying any concerns to other Prescribers and other medical staff/liliReports medication compliance to the clinical team/liliCoordinates directly with Residence Manager to ensure shift coverage for unapproved or unplanned absences./liliProvides Day to Day support of client wellness within the setting/liliObserves and identifies potential client medical or medication concerns/liliFacilities identification of medical concerns to appropriate medical or psychiatric staff or prescribers/liliAssures identified medical concerns are addressed/liliCommunicates and collaborates with appropriate Agency staff, Medical care providers, and related community organizations to assure coordination of client care and services./liliCompletes required paperwork related to communication or services provided/liliCoordinates with appropriate residential staff to facilitate transportation to necessary medical appointments/liliPromotes health and wellness within the residential facility/liliSupports the development of client wellness education and activities/liliAssists Medical Staff and Prescribers/liliProvides Support with vital signs, and other assistance to providers as needed/liliCollaborates with appropriate residential staff on client discharge plans/liliParticipates in staff meetings and medical/clinical supervision as directed/liliParticipates in relevant Agency meetings and/or trainings/liliActively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops./liliAwareness and active support of the Agencys Mission, Vision, Values and Strategic Plan. Valid Licensed Practical Nurse required. Two to three years of experience related to nursing services to individuals with mental health, alcoholism, chemical dependency, substance use disorder related services preferred.
