Key Responsibilities\n

In this role, your day-to-day responsibilities will include:

• Collaborating with our rehabilitation team to create comprehensive, goal-oriented treatment plans that take into account each patient's psychosocial needs.

• Guiding patients in navigating community resources, such as support groups, vocational training, and housing assistance, to ease their transition back into everyday life.

• Facilitating patient education about post-rehabilitation resources and community support options to ensure sustained emotional and practical assistance.

• Taking part in discharge planning processes to support smooth transitions for patients returning home or moving to alternative care settings.

The patients you will work with come from diverse backgrounds and include individuals recovering from various conditions such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n
