We offer nationally accreditedtoddler, preschool, afterschool, and summer programs for children aged 15months to 12 years old, as well as adult day care services that allow moreadults in our community to age in place. Position Overview The Family Advocateplays apivotal role in assisting families facing economic hardship, housinginstability, and other challenges. This position provides case managementservices, connects clients with community resources, and advocates for theirwell-being. TheFamily Advocateworks collaboratively with familiesto develop individualized action plans that promote long-term stability andself-sufficiency. Duties And Responsibilities Primary Responsibilities: The Family Advocate works closely across departments to help support the mission of Neighborhood House to empower, educate and enrich the lives of families through quality, affordable day care, preschool, adult days services and support services for low-income families. W orks directly with families in the Neighborhood House Children’s and Adult Programs to support their overall success. Connects with and assesses the needs of families and coordinate support programs and services to meet their financial, employment, educational, social and emotional needs. Oversees creating and maintaining reciprocal partnerships with services and organizations in the community, and high-level relationship management. Assesses the eligibility for appropriate assistance and guide families on application process, requirements and benefits of determined program. Including but not limited to Childcare Subsidies, CHIP, Medicaid, rental assistance. Assists parents/caregivers withunderstanding and implementing the goals for hardship assistance torestabilize their financial situation while keeping their loved ones inservices. Coordinates with partnershiporganizations to offer resources, workshops, and support groups forparents and caregivers. Maintain system for datamanagement, outcome measurements and statistics. Works in collaboration withother program staff and consultants to monitor, track and coordinateservices for children, adults and caregivers. Refers and links clients andcaregivers to needed services; ensures that referrals are being followedthrough on by families and documents their efforts to do so. Manages partnerships withcommunity organizations. Compliance with all licensing,HIPPA, best practice and accreditation regulations. Supports our Caring Closet,Resource Wednesday, Cyber Center and Family Fun Nights. Qualifications: Current licensed social service worker (SSW) with LCSWpreferred or equivalent Culturally andlinguistically diverse with Spanish fluency required Minimum2 years of experience with case management. Extensiveknowledge of local community resources and capacity to develop strongpartnerships to ensure that families receive comprehensive services Strongwritten, verbal, communication and organizational skills Capableof managing multiple priorities and meeting deadlines Ability to workevenings and weekends as required.
