Overview:

Family Support Program (FSP), formerly known as the Individual Care Grant program, provides access to intensive mental health services and supports to youth with a severe emotional disturbance.  The goal of the FSP is to support eligible youth and their families by strengthening family stability, improving clinical outcomes, and promoting community-based services.

The Specialized Family Support Program (SFSP) is a 90-day program of crisis stabilization, community mental health, and assessment services.  It is designed to identify the behavioral health needs of youth at risk of custody relinquishment (i.e. psychiatric hospital lockout) and to link those youth to the most appropriate clinical services.  

Basic Job Function:

Under the supervision of a Licensed Practitioner of the Healing Arts (LPHA), may provide some or all of the following: clinical assessment, treatment planning, crisis intervention, coordination with the Illinois Department of Healthcare and Family Services (HFS) and residential entities, collaboration with hospital staff, case management, community support, therapy/counseling, and administer direct clinical interventions to children, adolescents, and their families experiencing acute and chronic emotional/behavioral disorders. The FSP/SFSP Coordinator will provide these services in a variety of settings - primarily within the home or community.

Principal Duties/Responsibilities:

  • Working knowledge of FSP criteria and requirements. Ability to communicate and coordinate with potential FSP candidates in order to enroll them into the program. Once approved, maintains their enrollment and links the recipient to community based and/or residential based services. Maintains weekly verbal communication with client and family to ensure stabilization goals are being met and maintained. Completes and submits continued enrollments, prior authorizations, and applications as needed in accordance with HFS protocol.
  • Working knowledge of SFSP criteria and requirements. Facilitates in enrolling youth into the program, gathering all required documentation and coordinating with the hospital staff until the youth is discharged into the parents’ care. Provides support and direct services to the family for 90 days in order to maintain stabilization in the home. Completes required HFS reports.
  • Performs Integrated Assessments and Treatment Plans (IATP) for children and adolescents referred for FSP/SFSP. This includes the ability to develop and successfully provide a diagnostic assessment consistent with all Axis of the current Diagnostic and Statistical Manual and International Classification of Diseases, or their successors. Assures that all services provided are in compliance with standards of informed consent, maintenance of client rights, and standards of confidentiality.
  • Maintains direct service implementation included in the client's treatment plan. Engages client in clinical therapeutic rehabilitative services, case management, community support, crisis intervention, and mental health stabilization.
  • Provides consultation with other professional service providers in accordance with the current administrative code.
  • Prepares and oversees case presentation for clinical review at specified points of service, including initial assessments, treatment planning review, treatment staffing reviews and case closure.
  • Assures that all clients are linked with appropriate treatment, educational, social, recreational, and medical resources during and prior to completion of service.
  • Maintains current knowledge regarding services to children, adolescents, adults, and families. Participates in agency in-service trainings, as well as self and supervisor directed advanced skill-trainings. Maintains and adheres to current rules and regulations pertaining to services delivered by the Illinois Department of Human Services, Department of Children & Family Services, Illinois Healthcare & Family Services, including the Abused and Neglected Child Reporting Act (ANCRA), and Office of the Inspector General.
  • Participates at an advanced level with colleagues, agency consultants, intra and inter-agency service coordination, and community-planning and advocacy on behalf of clients served.
  • Documents service provisions consistent with the treatment plan and assures that all identified services are provided in accordance to Agency/Program, and current administrative codes for the delivery of community mental health treatment services. Maintains client confidentiality and adheres to guidelines defined by Health Insurance Portability and Accountability Act (HIPAA).
  • Performs other duties commensurate with the responsibilities outlined herein.
  • Maintains current and accurate documentation of client services (billable/non-billable), client scheduling, consultation, and other events related to the maintenance of the client's electronic health record.

Specifications:

This position requires a minimum of a master's degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, family therapy, or a related field as defined by the governing mental health authority. 

Must have successfully completed a practicum and/or internship which includes a minimum of 1,000 hours, or one year of clinical experience under a Licensed Qualified Mental Health Professional, including QMHP physician's, licensed clinical psychologists, or registered nurses licensed under the Illinois Nursing Act of 1987 with at least one year of clinical experience in a mental health setting.

The position requires the use of a personal auto, valid driver's license, and Motor Vehicle report acceptable to the agency's broker and verification of insurance coverage.

Benefit options and eligibility vary by Fulltime and Part-time positions. Compensation within the posted salary range varies based on factors including, but not limited to, experience, skills, education, and performance at the time of the offer .

Mental/Physical Demands: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel and talk or hear. The employee frequently is required to sit. The employee is occasionally required to stand and walk. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Environmental Conditions: The noise level in the work environment is usually moderate. Duties are performed in a typical office environment. No unusual or adverse working conditions exist.
