Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, serving as an integral part of the interdisciplinary healthcare team.

• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations to recommend individualized treatment goals and plans effectively.

• Provide brief yet impactful interventions for clients requiring mental health support while ensuring alignment with established Agency and Federally Qualified Health Center (FQHC) guidelines.

• Advise healthcare teams on patient referrals to specialty mental health facilities, facilitating seamless transitions for further care when necessary.

• Initiate regular follow-ups to monitor patient progress, adjusting treatment approaches as needed to optimize clinical outcomes.
\n
• Develop and implement relapse prevention strategies tailored to individual patient needs, promoting long-term stability and wellness.

• Detect at-risk patients and create actionable plans to prevent deterioration of their mental health status.
\n
• Effectively document patient progress, treatment plans, and diagnostic information in electronic health records (EHR), maintaining accuracy and confidentiality.

• Provide valuable self-management skills and educational resources to empower patients in their treatment and recovery journey.

• Assist in the coordination of behavioral health services, linking patients with other treatment providers both within and outside the primary care setting.

• Stay informed about community resources that can support patient care and collaborate with clinic management on service evaluations and enhancements.

Qualifications\n

• A minimum of a Master's Degree in Clinical Counseling or a related field.

Compensation and Benefits\n

Oasis Psychotherapy Services offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
