Hospice Patient Intake & Care Specialist

Hospice & Palliative Care Buffalo in Buffalo, NY is seeking a Patient Services Specialist (Intake) to coordinate intake, scheduling, and care delivery for patients. You will conduct interviews, collect intake information, and route referrals to appropriate service providers under the Admission Team Manager. The role requires a high school diploma and healthcare experience; hospice experience is helpful. Normal hours are 8:30am–4:30pm, Monday through Friday, with weekend and holiday rotation as #J-18808-Ljbffr
