Key Responsibilities\n

• Provide high-quality psychotherapy and related mental health services to patients with various mental health conditions, including anxiety, depression, and PTSD.

• Conduct comprehensive assessments of patients’ emotional, behavioral, and social functioning to inform treatment decisions.

• Develop, monitor, and modify individualized treatment plans based on evidence-based practices and the unique needs of each patient, ensuring cultural competence and sensitivity.

• Collaborate closely with interdisciplinary healthcare teams, including physicians, nurse practitioners, and hospice staff, to ensure seamless and coordinated patient care.

• Regularly review patient progress, utilizing feedback to adapt treatment strategies and optimize outcomes.

• Educate patients and their families about mental health conditions, treatment options, and available community resources to support their journey.

Patient Population / Client Focus\n

The LCSW will work primarily with patients receiving hospice care, many of whom are dealing with significant psychological burdens related to terminal illness. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
