Key Responsibilities\n

• Develop, coordinate, and implement individualized support plans to assist clients in overcoming challenges and dependencies associated with end-of-life care.

• Conduct initial and ongoing assessments of clients by interviewing service users to identify their needs, strengths, and areas for improvement.

• Design tailored, measurable goals for each case based on the initial assessment and ongoing interactions with the clients.

• Provide continuous monitoring and evaluation of clients’ progress, adjusting treatment plans as necessary to ensure they align with each client's evolving needs.

• Offer guidance, information, and counseling during therapy sessions, helping clients make informed decisions regarding their care.

• Maintain accurate and confidential records of clients’ services, reporting their status and progress to appropriate team members.

• Attend and participate in case conferences to provide insights and collaborate with other professionals involved in the clients’ care.

• Act as a primary contact for clients, building and nurturing relationships with multidisciplinary teams to enhance care delivery.

• Connect clients with community resources to facilitate ongoing support and assist in their recovery journey.

Qualifications\n

Compensation and Benefits\n
