Key Responsibilities\n

• Provide therapeutic interventions through psychotherapy or counseling to individuals, families, and groups, specifically tailored to the needs of hospice clients.

• Conduct comprehensive interviews to gather pertinent social, personal, and health-related information, ensuring a thorough understanding of each client's unique situation.

• Evaluate mental health and emotional issues within your scope of practice and develop effective treatment plans accordingly.

• Design and implement programs and activities to address the social and emotional requirements of clients receiving hospice care.

• Facilitate cases involving cognitive, emotional, and behavioral health concerns, focusing on crisis resolution and connecting clients with community resources.

• Establish and nurture a therapeutic relationship predicated on trust, respect, and acceptance to enhance client engagement.

• Regularly assess and document the effectiveness of treatment services, updating client records in accordance with established protocols.

• Maintain an organized caseload and ensure that documentation is current and reflective of provided services and client progress.

• Offer individual clinical counseling for clients navigating end-of-life issues, grief, and other emotional challenges.

• Complete additional tasks as assigned to support team and organizational objectives.

The ideal candidate will focus on hospice patients, their families, and caregivers, providing mental health support during some of life's most challenging moments. Qualifications\n

• Master's degree in social work, counseling, psychology, or a related field.

• Current licensure as a Clinical Social Worker (LCSW) in the state of Texas.
