Work Environment\n

This role is entirely remote, allowing flexibility for our team members while still providing critical support to families in need. As a part of a hospice setting, you will be working with compassionate professionals dedicated to holistic care. While the work is performed from a remote environment, you will have the necessary access to tools and resources to engage with families effectively.

Key Responsibilities\n

Your role as an LCSW at Empower Well Therapy will involve a variety of essential functions, including:

• Conducting comprehensive evaluations and assessments of children and families to identify needs and outcomes.
\n
• Participating actively in the development and implementation of Individualized Family Service Plans (IFSP).
\n
• Collaborating within a multidisciplinary team to monitor family and child progress, adjusting strategies as needed.
\n
• Providing direct support to families to enhance their child's development by utilizing evidence-based practices.
\n
• Establishing strong rapport with children, families, and team members to foster a supportive and effective learning environment.
\n
• Adhering to all Federal and State regulations, ensuring the safety and health of staff and clients.
\n
• Utilizing coaching models and natural learning environments to deliver the highest quality of intervention.

Patient Population / Client Focus\n

Your focus will primarily be on young children with disabilities or developmental delays and their families. Through our Children's Services, you will guide these children, facilitating their growth in physical, cognitive, social/emotional, and communication skills during the crucial years of early development. Your expertise and empathy will be pivotal in navigating the complex challenges faced by families, working collaboratively to implement strategies that promote long-term success.

Qualifications\n

To be considered for this role, you must possess:

• A Master's Degree or Doctorate in Social Work.

• Licensure by the Department of Public Health as a Clinical Social Worker (LCSW).

• A valid driver's license, enabling travel to client locations if necessary.
\n
• Outstanding organizational and time management skills to manage your caseload efficiently.

• Excellent communication skills to foster collaboration with families and team members.
\n

Preferred Experience\n

While a minimum of one year of direct experience working with children in early intervention services is required, the following qualifications are preferred:
