Contract | Days | 8:00 AM – 4:00 PM or 9:00 AM – 5:00 PM | Onsite

Key Responsibilities

• Complete patient assessments within departmental timeframes, incorporating biopsychosocial, financial, medical, and physical factors alongside age-specific and clinical considerations
• Develop and coordinate safe discharge plans in collaboration with the multidisciplinary team, including post-hospital care options and placement in alternative healthcare facilities when appropriate
• Assess patient capacity for self-care and evaluate the availability of family or support networks to assist with care at home
• Provide short-term counseling for crisis intervention, problem solving, bereavement, and adjustment to medical diagnosis or hospitalization
• Advocate on behalf of patients with referral and payor sources, and coordinate services with payors, community resources, and the care team
• Comply with required standards for medical/legal reporting situations, including child abuse, adult abuse, and sexual abuse reporting

Required Qualifications

• Active Licensed Master of Social Work (LMSW) licensure in New York State required
• Minimum 2 years of experience in social work
• Experience working in a hospital setting required
• Demonstrated knowledge of community-based resources and discharge planning processes
• Ability to manage crisis situations, set priorities, and exercise effective time management in a fast-paced clinical environment

Preferred Experience / Skills

• Experience with Powerchart electronic health record system
• Familiarity with complex psychosocial and discharge planning cases
• Experience with multidisciplinary team collaboration and care coordination
