Job Description

As an Inpatient Advance Pharmacy Technician specializing in medication histories, you will perform critical functions to ensure accurate and complete medication documentation for inpatient care.

Key Responsibilities:

• Medication History Review: Systematically review office visit documentation, hospital discharge summaries, and pharmacy records to compile comprehensive medication histories and identify any gaps in documentation.
• Patient and Family Interviews: Conduct structured interviews with patients and family members to verify current medications, dosages, and administration instructions. Document all findings accurately and completely.
• Medication Verification: Verify medication names, dosages, frequencies, and administration instructions with precision. Review all medication categories including prescription drugs, over-the-counter supplements, sample medications, and as-needed medications.
• Discrepancy Resolution: Identify and resolve medication history discrepancies and incomplete information by contacting pharmacies and healthcare providers to obtain accurate data and ensure continuity of care.
• Electronic Medical Record Documentation: Update medication lists in the electronic medical record and document findings in patient progress notes. Obtain pharmacist verification that medication histories are accurately updated prior to provider notification.
• Controlled Substance Handling: Work under pharmacist supervision to process medications requiring special handling, preparation, and accountability. Maintain expertise in the safe management of controlled and specialized medications.
• Inventory Management: Maintain and stock pharmacy inventory while delivering prescriptions to nursing units to ensure operational efficiency and medication availability.

Qualifications

EDUCATION & EXPERIENCE:

• Requires a high school diploma or G.E.D. Requires a valid Pharmacy Technician license from the State of Michigan.
• Requires at least one year of experience as a Hospital Pharmacy Technician and/or Retail Pharmacy Technician (retail experience applies only to medication history positions.
• Requires the ability to maintain records and perform calculations. Must have the ability, when applicable, to safely handle and manipulate cytotoxic and other special drugs and incidental equipment used in drug preparation, including the ability to work aseptically in a vertical flow hood environment.
• Must have the ability, when applicable, to maintain Investigational Drug Research Records and to validate doses ordered using Investigational Drug Protocols.
• Must have the ability to maintain (lift, carry, stock) pharmacy inventory and deliver prescriptions (transport) to nursing units. Must have the ability to use a computer keyboard and type approximately 25 WPM.
• Must be able to read, interpret and utilize literature relating to medications that require special handling, preparation or accountability to develop or modify, in consultation with a supervising pharmacist, departmental protocols pertaining to the use of these agents.
