We are seeking a compassionate and skilled Memory Care Coordinator to join our team at Grand Villa Assisted Living in Winston-Salem, North Carolina. As a leading provider of assisted living services, we are dedicated to delivering exceptional care and support to our residents. Our Memory Care Coordinator will play a critical role in ensuring the well-being and quality of life of our residents living with memory impairments. MUST HAVE A NORTH CAROLINA MEDICATION TECHNICIAN CERTIFICATION.

About the Role:

As a Memory Care Coordinator , you will be responsible for coordinating and delivering personalized memory care services to our residents. This includes:

• Developing and implementing individualized care plans in collaboration with the care team

• Providing emotional support and guidance to residents and their families

• Facilitating resident activities and socialization programs

• Collaborating with caregivers and other healthcare professionals to ensure seamless care transitions

• Maintaining accurate and up-to-date records of resident care and services

Responsibilities:

• Coordinate and deliver memory care services, including medication management, behavioral interventions, and daily living assistance

• Develop and maintain relationships with residents, families, and care staff to ensure effective communication and collaboration

• Participate in interdisciplinary team meetings to discuss resident care and progress

• Identify and report any changes in resident condition or concerns to the care team

• Maintain a clean, organized, and safe environment for residents

Requirements:

• High school diploma or equivalent required; degree in a related field (e.g., gerontology, healthcare administration) preferred

• Minimum 0 years of experience in a long-term care or memory care setting

• Excellent communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
