Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
• Complete comprehensive evaluations using appropriate assessment tools and procedures
• Review patient records for current diagnosis, past medical history, precautions, and contraindications
• Establish measurable goals and develop treatment plans to enhance patient independence and function
• Regularly re-assess treatment plans and patient progress, modifying goals or treatment as needed
• Communicate patient needs and progress to the treatment team, physicians, and family members
• Consult with medical providers regarding patient evaluation, treatment, and discharge plans
• Assist in securing special adaptive equipment for patients to improve function
• Maintain and submit documentation according to company, facility, state, and federal regulations
• Adhere to the Company's Code of Business Conduct, policies, and legal standards
• Supervise Physical Therapist Assistants and Aides in direct patient care and related activities
• Participate in internal staff development programs and maintain clinical practice skills

Requirements of the Physical Therapist

• KY Physical Therapist Licensure, Compact Licensure Accepted
• 1 year of home health physical therapy experience
• Previous OASIS experience
• Experience with Home Care Home Base
