Connections Health Solutions is seeking a Case Manager for the RESET Center in San Francisco. This full-time role focuses on providing person-centered care coordination for individuals in crisis, ensuring they receive the support and resources needed for recovery. The ideal candidate has experience in harm reduction and behavioral health settings, along with necessary certifications.
