Home Health RN Case Manager

Your Health Inc

Your Health is seeking a Registered Nurse to service patients throughout the Brunswick area as a Visiting Nurse/Case Manager. The role involves traveling to patients’ homes and facilities to evaluate care needs, initiate telemedicine visits, and coordinate with providers while adhering to laws and policies. This full-time position runs 8-hour shifts, Monday–Friday, 8am–5pm. Ideal candidates will have RN licensure (RN preferred), strong communication skills, and at least three years of clinical #J-18808-Ljbffr
