Care CoordinatorCare Runners Home Care Services, Inc. is seeking a dedicated Care Coordinator to join our team in McDonough, GA. The ideal candidate will have experience working with clients with developmental disabilities and a strong person-centered focus to support our clients effectively. This role involves coordinating care plans and services tailored to individual needs, ensuring quality and compassionate care for our clients.ResponsibilitiesCoordinate and oversee care plans for DBHDD clients receiving home care services, ensuring they meet individual needs and preferences.Collaborate closely with clients, families, and healthcare providers to deliver person-centered care.Monitor client progress and adjust care plans as necessary to maintain high standards of care and client satisfaction.Maintain accurate and timely documentation in compliance with company policies and regulatory requirements.Serve as a liaison between clients, care staff, and external agencies to facilitate communication and service delivery.Participate in team meetings, training, and continuous quality improvement initiatives.RequirementsExperience working with clients with developmental disabilities and knowledge of behavioral health and developmental disability services.Strong commitment to a person-centered approach in service delivery.Excellent communication, organizational, and interpersonal skills.Ability to manage multiple tasks and coordinate care effectively.Proficiency with documentation and care coordination software preferred.High school diploma or equivalent required; additional education or certifications related to healthcare coordination are a plus.BenefitsCompetitive hourly pay of $16.00 - $18.00 per hour, paid bi-weekly.Opportunities for professional development and training.Supportive and collaborative work environment.Potential for growth within the company.About the CompanyCare Runners Home Care Services, Inc. is a dedicated provider of compassionate and quality home care services based in McDonough, GA.
