The bonus will be paid to the hired candidate half after 30 days ($2,500) and half upon the successful completion of 6 months of work ($2,500). ACT works as a team of clinicians who provide intensive, recovery oriented and community based behavioral health services to persons living with serious and persistent mental illness. Work closely with Front Door, Colbert and Williams teams, as well as other agency teams. Ensure appropriate clinical interventions and following fidelity treatment model. Participates in the program planning, development and achievement of team-specific budget, strategic goals, and initiatives alongside Director leadership. Responsible for stable hiring, effective training and support, and communication of management directives and information to staff. Program Responsibilities Oversee overall operations for the ACT team to assure clients in need of intensive, recovery oriented services are being served Ensure that care provided integrates all facets of clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use issues, and housing needs Focused support and development to expand team-based intensive mental health services in the Northwest Suburbs to increase capacity, increase service area, and open to internal referrals Monitor staffing needs and lead in the hiring and orientation/training of new staff Oversee Nurse Case Management services provided to Team Services (ACT & CST) Create and maintain ACT services manual Manage referrals and monitor team capacity Coordinate with Managers of CST, Front Door, Community Therapy, PSH, and Colbert & Williams Transition teams to ensure transition of Colbert, Williams, and Front Door referrals into aftercare services Monitor team compliance with reporting and ensure Colbert, Williams, and Front Door requirements are met during aftercare service provision Act as a liaison between agency departments (IRG, SUPR, medication monitoring, Older Adult, Intake, Employment, etc) to ensure service coordination and continuity of care Oversee and maintain program fidelity Participate in Continuous Quality Improvement (CQI) committee, utilization and peer review Provide weekly scheduled and ad-hoc clinical and administrative supervision of team supervisors, as well as direct-service staff when needed Regularly monitor and evaluate progress towards individual and team performance goals and monitor documentation quality and compliance Ensure that all clinical activities performed by the team are compliant with standards, policies and procedures of the organization Participate in department budget process, actively monitor team member’s productivity, and participate in monthly finance meetings to assure Identify opportunities and develop relationships with area providers to best meet community member’s intensive mental health needs Coordinate and participate in 24/7 on-call scheduling and backup manager supports Provide in-office support as manager on-site and in-community support on a regular weekly basis Other duties as assigned Individual Performance Participate in clinical/administrative supervision; seek individual consultation with Deputy Director when needed. Meet or exceed individual productivity expectation Accurately complete clinical documentation and service reporting in a timely manner Participation in program planning, service coordination, practice development and quality assurance as a member of PSC (program services committee) and CQI (continuous quality improvement) committee Participate in monthly manager and department leadership meetings Participate in orientations, all staff meetings and training/ development Interpret and communicate, at the service delivery level, Center policies and procedures Submit, in a timely and accurate fashion, schedules, timesheets, mileage and other reports required by the Center’s policies and procedures Review supervisees clinical work and IATP (integrated assessment and treatment plans) Complete performance evaluations for all supervisees building on strengths and identifying ongoing growth/training needs Education & Experience Master’s Degree from an accredited college or university in social work, psychology or a related discipline to meet Illinois QMHP designation LCPC/LCSW or equivalent that meets IL Rule 132/140 LPHA designation strongly preferred 5 years of experience working with individuals with serious mental illness 2 years working in non-traditional treatment environments i.e.in-home, residential, hospital, community settings Preferred experience providing team-based ACT or CST services Management and/or development of mental health programming At least 2 years of demonstrated ability to provide clinical andadministrative supervision IM-CANS certified or obtain within 45 days Basic technology, smartphone, and computer proficiency; knowledge of web-based team communication tools (ex. Microsoft Teams) and Word, Excel, and internet skills Work Environment / Schedule While performing the duties of this job, the employee regularly works in an office setting at various KYC office locations in Northwest suburbs Will be required to drive and attend meetings, training, etc. in and around the catchment area At least 25% of employee’s time will be spent in the community, driving to and from locations, shadowing and training staff, support crisis activities, and providing mental health services in the home or community areas In supporting clients to live independently, employee may be exposed to varying home conditions and drive clients in their vehicle Opportunity for a portion of the week to include Work from Home with Director/Deputy Director approval. This schedule and approval is subject to change based on the needs of program Full Time, Exempt, 37.5 hours per week Majority of schedule during workweek business hours and must be prepared to work a flexible schedule in response to the needs of the target population served, including evenings and/or weekends.
