RN Case Manager
Coordinates and oversees patients' individualized care plans, collaborating with physicians, therapists, nurses, patients, and families to facilitate rehabilitation goals, manage resources, monitor progress, and ensure a safe and efficient transition to the next level of care.
Minimum Required Qualifications:
- CO/compact RN license
- BLS
- 1+ year recent case management experience in an acute hospital setting
Length of Assignment: 13 weeks
Shift / Hours Per Week: 8:00 AM-4:30 PM. Weekend Hours: 1-5.
Systems: Epic
Start Date: ASAP
