RN / NP – HOSPICE & PALLIATIVE CARE

FULLY REMOTE
$60/HOUR
OPEN ENDED CONTRACT

We are seeking experienced Registered Nurses (RNs) or Nurse Practitioners (NPs) with a strong background in Hospice, Palliative Care, or Advanced Illness Management.

Ideal candidates will have experience providing Care Management or Care Coordination for patients with advanced or serious illnesses.

QUALIFICATIONS

Active New York State RN or NP license – REQUIRED
Minimum 3–5 years of experience in Care Management, Hospice, Palliative Care, Advanced Illness Management, or a related field
Strong knowledge of Advanced Care Planning
Experience with Goals of Care discussions
Strong understanding of end-of-life care principles
Care Management and Care Coordination experience
Associate’s Degree in Nursing required
Bachelor’s Degree preferred
CHPN certification is a plus

Candidates with Advanced Illness Management, Palliative Care, or Hospice experience in a Care Management or Care Coordination capacity are strongly preferred.

PAY: $60/HOUR

100% FULLY REMOTE
