Purpose and Scope Functions as part of the Home Therapies dialysis health care team. Ensures provision of quality home patient care in accordance with company policies, procedures and training. Supports the company’s commitment to the Quality Enhancement Program (QEP) and CQI activities, including those related to patient satisfaction. Actively participates in process improvement activities that enhance the likelihood that patients will achieve the company’s Quality Enhancement Goals. Principal Duties and Responsibilities Staff Related: Provide safe and effective training and oversight of the delivery of home dialysis therapies to patients in compliance with facility policy, procedure manuals and regulatory standards. Rotate with other licensed staff to ensure adequate coverage, participate in on‑call rotation and staff training, orientation of new staff, and all required staff meetings. Quality: Provide outstanding quality of patient care as defined by company quality goals and standards; submit CQI information for HT QAI template; participate in all monthly CQI meetings. Education: Assist in identification and evaluation of candidates for the Home Therapies Program and assessment of patients’ home environment. Train home patients and families on safe and effective operation of equipment, educate on ESRD and the Kidney Options Program, assess readiness and ability to perform dialysis, and provide ongoing education regarding dialysis, access, and related health conditions. Dialysis Treatment: Deliver safe and effective home therapy support care; assess patients’ response by following predetermined protocols; communicate patient issues to physicians; report adverse patient events and equipment problems; coordinate transfer to hemodialysis clinic when needed. Laboratory Related: Review, transcribe, and enter physician lab orders accurately into the Medical Information System; follow company policies and regulations for specimen handling and reporting. General Duties: Maintain competency with emergency procedures and CPR; administer medications as prescribed; document medical justification when required; maintain records on controlled substances. Maintenance/Technical: Maintain skill level in operating all home dialysis equipment, troubleshoot equipment problems via telephone, direct patients in preventive maintenance, and coordinate with the products division for equipment delivery, operation, and maintenance. Medical Records & Documentation: Admit new patients; document orders, lab results, vitals, treatment parameters, and patient status; ensure all company policies regarding admission, transfer, and discharge are implemented; complete required monthly and annual reporting. Other: Perform other related duties as assigned. Physical Demands and Working Conditions Day‑to‑day work includes desk and personal computer tasks, interaction with patients, facility staff, and physicians. The position requires travel between assigned facilities and various locations within the community, including regional, business unit, and corporate meetings, as needed. Work is conducted in a health care facility with controlled air temperature and moderate noise levels. Must interact safely with infectious and contagious materials and complete the Ishihara Color Blindness test as a condition of employment. Education and Required Credentials Graduate of an accredited School of Nursing (RN). Current appropriate state licensure. Experience and Skills Minimum one year of medical‑surgical nursing experience (ICU experience preferred). Minimum one year of dialysis experience (peritoneal experience preferred). Successful completion of a training course in the theory and practice of dialysis therapies. Competence in operating all Home Therapies related equipment. CPR certification with maintenance as required. ICD‑9 coding training. Demonstrated teaching abilities.
