Job Description

We are seeking a qualified Travel RN Case Manager to join our dynamic team. This position requires expertise in case management, specifically focusing on cardiac and medical step-down units. The ideal candidate will work collaboratively with physicians and mid-levels in a busy and fast-paced environment.

Responsibilities

• Conduct case management assessments and discharge planning
• Collaborate with a multidisciplinary team to ensure quality patient care
• Manage patient care efficiently in the CPCU and PCU settings

Required Experience / Certifications / Licensure

• Minimum 1 year current acute care Case Manager RN experience
• RN license or Licensed Social Worker in Kansas
• BLS certification
• Meditech and Midas experience preferred
