This full-time remote position is based within a Detox Center located in Murfreesboro, Tennessee. Work Environment\n

This position is fully remote, allowing for flexible work arrangements. While the standard work hours are Monday through Friday, from 8:00 AM to 4:30 PM, you may occasionally be required to work evenings or weekends based on client needs. Your flexibility in scheduling is crucial to provide ongoing care and support to our clients, particularly during urgent situations that may arise outside of regular working hours.

Key Responsibilities\n

• Conduct comprehensive assessments to identify the psychosocial, emotional, and mental health needs of clients, particularly seniors, ensuring that all factors affecting their well-being are considered.

• Collaborate with clients and their families to create individualized care plans that align with their specific goals and preferences, facilitating a personalized approach to their mental health journey.
\n
• Provide counseling and emotional support to clients, guiding them through various life transitions, including recovery from substance use, family changes, and coping with grief.

• Advocate for clients, ensuring they have access to essential community resources, services, and rights within the healthcare system.
\n
• Work alongside an interdisciplinary team including healthcare professionals, nurses, and caregivers to coordinate and enhance client care, contributing to holistic treatment plans.

• Monitor clients' conditions through regular follow-ups, ensuring that their living environments are safe and conducive to well-being, while addressing any emerging concerns.

• Facilitate family communication, mediating discussions to resolve conflicts and enhance collective understanding of client needs.

• Educate clients and families on available support resources, community programs, and groups that can improve their social engagement and overall quality of life.

• Maintain detailed and accurate documentation related to client assessments, progress notes, and intervention strategies to comply with best practices and licensing requirements.

• Stay current with developments in social work practices and engage in continuing education to uphold high standards of ethical practice.

Patient Population / Client Focus\n

Your primary focus will be on seniors and individuals undergoing detoxification, many of whom face significant emotional and mental health challenges. You will provide tailored support to not only help them navigate their immediate needs but also equip them with effective coping strategies for long-term mental health stability and resilience.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is mandatory.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee is required.

• A minimum of two years' experience in a social work role, preferably in a community setting or with geriatric populations, highlighting practical skills in assessment and intervention.

• In-depth knowledge of social work principles, particularly related to aging issues, mental health concerns, and community resource networks.

• Strong assessment, counseling, and problem-solving abilities, complemented by a capacity to empathize and build meaningful rapport with clients and families.

• Excellent communication and teamwork skills that facilitate effective collaboration with multidisciplinary teams.

• Familiarity with electronic health record (EHR) systems and other relevant software applications to document client interactions and maintain records accurately.
