Work Environment

This position is a full-time role based onsite, directly in our patients' homes. You will work closely with interdisciplinary teams composed of physicians, nurses, occupational therapists, and speech-language pathologists. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities, identifying limitations and functional impairments caused by illness, injury, or disability.

• Design and implement individualized treatment programs based on assessment findings, patient goals, and physician directives to maximize rehabilitation outcomes.

• Maintain accurate and timely documentation of patient evaluations, treatment plans, and progress notes, ensuring compliance with industry standards and organizational procedures.

• Monitor patient progress throughout treatment, making necessary adjustments to treatment plans to optimize recovery and functional independence.

• Supervise the activities of Physical Therapist Assistants (PTAs) and support staff involved in patient care to ensure high-quality support and adherence to care protocols.

• Collaborate with a diverse team of healthcare professionals to develop integrated care plans that address the holistic needs of each patient.

• Advise patients and their families on exercises, activities, and lifestyle changes that enhance recovery and promote long-term health.

• Participate in team meetings and professional development initiatives to stay current with best practices in physical therapy and adapt new methodologies.

Patient Population / Client Focus

The primary focus of this role is on patients who are experiencing physical impairments due to various conditions such as orthopedic injuries, neurological disorders, and other medical complications. You will work with patients across various age groups, tailoring your approach to meet age-appropriate developmental and therapeutic needs.

Qualifications

• Bachelor of Science degree in Physical Therapy from an accredited university.

• Current license to practice as a Physical Therapist in the state of Virginia.

• Strong written and verbal communication skills are essential to effectively engage with patients and collaborate with multi-disciplinary teams.

• Basic proficiency in computer skills is required for documentation and care coordination.

Preferred Experience

• A minimum of two years of experience in a physical therapy setting, particularly in home care or community-based services, is preferred.

Compensation and Benefits

The salary range for this position is competitive, between $80,000 and $100,000 annually, depending on experience.
