We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Mosholu Parkway Nursing & Rehabilitation Center in The Bronx, New York. As a valued member of our staff, you will have the opportunity to work with a diverse group of patients, providing high-quality care and making a meaningful difference in their lives.
Responsibilities:
Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living
Collaborate with interdisciplinary teams to develop and implement patient care plans
Conduct vital signs, monitor patient conditions, and report changes to nursing staff
Participate in providing education and support to patients and families
Maintain accurate and up-to-date patient records
Assist with scheduling and documenting doctor's orders and treatments
Requirements:
Current LPN license in the State of New York
Minimum [insert required education and experience]
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
