Job Description – Home Hemodialysis or HHD Travel RN (Registered Nurse)

• Provide in-home training and clinical support to patients and caregivers learning to perform dialysis independently in a home setting.
• Assess patients' physical and emotional readiness for home dialysis, including suitability of the home environment.
• Deliver patient education and hands-on instruction covering dialysis procedures, infection prevention, machine operation, and emergency protocols.
• Evaluate patient competency throughout training and provide ongoing support post-training through telehealth or in-home visits.
• Collaborate with nephrologists, home program coordinators, dietitians, and social workers to build individualized treatment plans and monitor patient progress.
• Document training activities, patient assessments, and ongoing clinical status in the electronic medical record (EMR).
• Perform equipment setup, calibration, maintenance, and troubleshooting for home dialysis machines.
• Monitor patient adherence and intervene promptly if complications, concerns, or non-compliance arise.
• Ensure compliance with all safety, infection control, and quality assurance standards related to home hemodialysis.
• Provide emotional support to patients and families managing chronic kidney disease in the home care setting.
• Participate in continuous quality improvement (CQI) and program development initiatives.

Job Requirements

• 1+ year of recent experience as a home hemodialysis (HHD) RN
• Graduate from an accredited nursing school
• Current state specific nursing license in good standing, or if applicable, a compact state RN license in good standing
• BLS certification required (Chronic, HHD, and Acute settings)
• ACLS certification required (Acute setting only)
