Integrated Care Nurse PractitionerLocation: Tacoma / Greater Seattle Area, WashingtonSalary: $120,000–$145,000 annuallyWork Setting: Field-Based | In-Home CareJob OverviewThe Integrated Care Nurse Practitioner provides personalized, value-based medical care to patients living primarily with chronic kidney disease and end-stage renal disease. This field-based role includes in-home patient visits, comprehensive health assessments, care coordination, preventive management, and ongoing treatment planning.The Nurse Practitioner will work closely with physicians, field nurses, community health workers, primary care providers, specialists, and ancillary services to improve patient outcomes, close gaps in care, reduce avoidable hospitalizations, and enhance patients’ quality of life.This is not an office-based position. Candidates must be comfortable traveling throughout their assigned territory and should live in or near the Greater Seattle and Tacoma area.Preferred candidate locations include:TacomaNewcastleSeattle and South SeattleShorelineBellevueMedinaIssaquahRedmondTravel RequirementsCandidates must be comfortable traveling throughout the assigned territory for in-home patient visits.The position may also require quarterly travel for up to three days to locations approximately 200 miles away. Approved hotel accommodations and rental-car expenses may be reimbursed in accordance with company policy. The hiring team will provide additional travel details during the interview process.Key ResponsibilitiesConduct comprehensive annual wellness examinations in patients’ homes and through virtual appointmentsPerform health assessments and evaluate patients’ current health statusDevelop, implement, and evaluate individualized treatment plansPrescribe medications and order diagnostic testing as appropriateEducate patients and families on disease management, preventive care, healthy lifestyle habits, and available programsCoordinate care with primary care providers, specialists, physicians, and ancillary servicesIdentify and close gaps in care through preventive monitoring, patient education, and provider collaborationDocument complete patient histories, examinations, diagnoses, medications, allergies, assessments, and care plansComplete all clinical documentation and required paperwork accurately and on timeDeliver evidence-based care designed to reduce avoidable hospitalizations and improve patient outcomesParticipate in integrated care team meetingsConnect patients with educational materials, support programs, and healthcare resourcesMaintain established standards for quality, compliance, and patient satisfactionPerform additional duties as assignedRequired QualificationsActive and unrestricted Washington Registered Nurse and Nurse Practitioner licensesCurrent Nurse Practitioner board certification through the ANCC or AANPCurrent and unrestricted DEA certificateMinimum of two years of direct patient care experienceAbility to practice autonomously without direct supervisionValid driver’s license, reliable transportation, and current automobile insuranceAbility and willingness to travel throughout the assigned territoryProficiency with electronic medical records, medical equipment, information systems, and point-of-care technologyStrong verbal and written communication skillsKnowledge of HIPAA, OSHA, and applicable federal and state healthcare regulationsPreferred QualificationsManaged care, Independent Practice Association, or health plan experienceExperience conducting annual wellness visits or comprehensive assessments in patients’ homes or virtuallyExperience working with chronic kidney disease, end-stage renal disease, or other complex chronic conditionsExperience in value-based care, population health, or care managementBilingual Spanish skillsBenefitsCompetitive salaryComprehensive medical, dental, vision, and life insuranceFlexible paid leave and vacation policy401(k) plan with employer matching contributionsOpportunity to join a fast-growing, innovative, value-based healthcare organization transforming the delivery of kidney and chronic-condition carePandoLogic.
