RN Case Manager
Good Samaritan
Good Samaritan Health Center in Atlanta seeks a compassionate RN Case Manager to coordinate whole-person care for underserved patients in our primary care setting. This role conducts clinical and psychosocial assessments, develops individualized care plans, and guides patients through medical, behavioral health, and community resources. The RN Case Manager collaborates closely with an interdisciplinary, faith-inspired team to reduce barriers to care, improve outcomes, and promote health equity.
