RN Case Manager
Good Samaritan
Good Samaritan Health Center seeks a compassionate RN Case Manager to coordinate holistic care for underserved patients in our faith-based, mission-driven clinic. The RN Case Manager will perform clinical assessments, develop and oversee person-centered care plans, facilitate transitions of care, and connect patients with medical, behavioral, and community resources. This role works closely with an interdisciplinary team to reduce barriers to care, support chronic disease management, and promote whole-person wellbeing while advancing health equity in a collaborative, service-oriented environment.
