RN Case Manager
Good Samaritan
Good Samaritan Health Center in Atlanta seeks an RN Case Manager to coordinate holistic care for underserved patients in a mission-driven, faith-based clinic. The RN Case Manager will complete comprehensive assessments, develop care plans, provide education and care coordination, and partner closely with primary care, dental, behavioral health, and community resources to address medical, social, and spiritual needs. This role supports care transitions, quality improvement, and health equity while working in a collaborative, team-based environment focused on whole-person wellbeing.
