RN Case Manager
Good Samaritan
Good Samaritan Health Center seeks an RN Case Manager to coordinate holistic, patient-centered care for underserved individuals and families in our faith-based, nonprofit clinic. The RN Case Manager conducts comprehensive assessments, develops and oversees care plans, coordinates services across medical, behavioral health, dental, and community partners, and provides education that empowers patients to manage chronic and acute conditions. This role actively addresses social determinants of health, removes barriers to care, and contributes to quality and population health initiatives within a collaborative, mission-driven team committed to health equity.
