We are looking for a full-time Care Coordinator for our Las Vegas, Nevada location!Compensation: $52,000.00-$55,000.00 (Salary commensurate with experience).Working At NYAPNYAP's commitment to doing what is best for children, youth and their families is a core value and one that we look for in our newest team members.Excellent training and continuing education and development opportunities offered on topics such as: PCIT, NMT, TF-CBT, BFST, CSAYC, TBRI, FFT and many, many more!Summer Hours (half day off on Friday's)Student Loan Repayment assistance, up to $1,200 per year!Medical, Dental, and Vision insurance for you and your family!22 Days Off Each Year!Plus 11 Paid Holidays Per Year!Competitive salaries and benefits including a 401(k)Tuition AssistanceWork Anniversary Trips!Peace leaveFlexible HoursPhone allowanceMileage ReimbursementPosition Summary:The Care Coordinator will aim to achieve the ultimate goal to keep youth in their homes, communities and schools by assessing and delivering the appropriate services needed and reducing unnecessary out-of-home placement and potential custody relinquishment.RESPONSIBILITIESThe Care Coordinator will perform duties including, but not limited to:Perform all work in a manner consistent with the National Youth Advocate Programs mission, values, and philosophies.Cultivate flexible, family-focused, community-based responsive services based on the High Fidelity WrapAround model of care coordination.Develop and maintain the Wraparound Team, including coordinating and leading team meetings.Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update plan as necessary.Ensure family support and stabilization during crises.Provide and document the initial and ongoing Life Domain Assessment.Maintain all service documentation requirements, evaluation outcome requirements and data as required.Provide services in a timely manner and in accordance with Plan of Care and/or Crisis Plan.Utilize and monitor Flexible Funding and service coordination.Obtain weekly reports from subcontracted providers.Participate in after hours on-call response.Attend Program staff meetings, supervision and any other meetings as required.Participate in the Agency and Program CQI Peer review process.Perform duties to reflect Agency policies and procedures and comply with regulatory standards.Meeting Agency training requirements.Report all MUl's to Site Manager and Supervisor immediately.Performs other duties as requested.Minimum Qualifications:Will complete the high-fidelity wraparound training programWill have experience providing community-based services to children and youth and their families or caregivers in areas of children’s behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field for:three years with a high school diploma or equivalent; ortwo years with an associate degree or bachelor’s degree; orone year with a master’s degree or higherHave background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.Be culturally competent or responsive with training and experience necessary to manage complex cases.Have the qualifications and experience needed to work with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders and who are engaged with one or more child- serving systems.
