Our Community Living Department is seeking a dedicated and dynamic Case Manager for our Supported Living Program, serving the San Francisco, San Mateo, and East Bay Areas. About the Role As a Program Case Manager, you will be at the heart of our mission, overseeing 6-8 Supported Living cases and ensuring that our clients lead fulfilling lives in the communities of their choice. Key Responsibilities Emergency Response: Manage and respond to client and staffing-related emergencies, ensuring safety and continuity of care. Client Engagement: Maintain regular, meaningful contact with clients and their support networks, fostering independence and community integration. Staff Leadership: Recruit, train, and mentor your team, ensuring they are equipped and inspired to provide exceptional support. Administrative Excellence: Oversee scheduling, training compliance, and safety procedures, ensuring smooth operations and the highest quality of service. Community Representation: Serve as a compassionate ambassador for Toolworks, upholding our values and fostering a positive community presence. What We're Looking For Education: A Bachelor's degree in Special Education, Rehabilitation Counseling, or a related field is preferred but not required. Experience: At least three years in a role supporting adults with disabilities, with a preference for those with case management experience. Supervisory experience is also valued. Skills: Exceptional report writing, strong time management, and a knack for recognizing and resolving challenges. Requirements: A clean DMV printout, First Aid/CPR certification, and the ability to travel within the designated counties are all essential.
