Key Responsibilities

• Conduct thorough assessments to diagnose acute and chronic medical conditions, creating individualized care plans tailored to each resident's needs.
• Implement evidence-based treatment plans in conjunction with a multidisciplinary team, including doctors and specialists, to enhance resident health outcomes.
• Order, perform, and interpret diagnostic tests, and prescribe appropriate medications and therapies in line with best practices.
• Educate residents and their families regarding disease management, medication adherence, and post-discharge care, empowering them to actively participate in their health journey.
• Lead interdisciplinary team rounds, discussing resident care plans and adjusting strategies as necessary to promote optimal health outcomes.
• Maintain accurate, detailed electronic medical records (EMR), ensuring compliance with health regulations and facilitating seamless communication within the care team.
• Engage in quality improvement initiatives, contributing to the development and updating of evidence-based practice guidelines.
• Keep abreast of the latest healthcare advancements and regulatory changes, incorporating new knowledge into clinical practice.

Patient Population / Client Focus

Our residents range from those requiring minimal assistance to individuals with complex medical needs. You will care for a diverse population, including elderly individuals with conditions such as dementia, heart disease, and diabetes. This role allows you to make a significant impact on their quality of life through compassionate and individualized care.

Qualifications

• Current certification as a Nurse Practitioner along with an active nursing license in the state of Texas.
• A Bachelor’s degree in Nursing is required; a Master’s degree in Nursing or an equivalent advanced practice degree is preferred.
• A minimum of three years of clinical experience, ideally in an assisted living or similar healthcare environment.
• Exceptional communication skills to foster collaboration with residents, family members, and the healthcare team.

Preferred Experience

Compensation and Benefits

We offer a competitive salary range of USD 120,000 to 220,000 annually, commensurate with your experience and qualifications.
