Evergreen Nephrology seeks an Implementation Manager for Evergreen Connected Care to drive onboarding and ongoing relationships with partner practices. You will own the end-to-end onboarding sequence, manage timelines, and coordinate with clinical and program leaders to scale the execution as the program grows. This fully remote role requires exceptional communication, multi-stakeholder coordination, and occasional travel to practice sites.
