Description

• Assess and evaluate patients in both inpatient and outpatient settings, designing personalized treatment regimens.
• Implement physical therapy interventions with a focus on enhancing mobility, strength, balance, and overall functionality.
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to optimize patient outcomes.
• Inform and educate patients and their families about therapeutic goals, discharge planning, and home exercise protocols.
• Keep thorough documentation of evaluations, treatments, and patient progress in accordance with facility standards and regulatory guidelines.
• Engage in interdisciplinary care team meetings and rounds to ensure cohesion in patient care.

Education

• Doctorate in Physical Therapy (DPT) or a related accredited degree.

Certifications

• Licensed to practice as a Physical Therapist in the state.
• Preferred CPR certification.
• Additional certifications in specialized fields (e.g., geriatric, neurological) are advantageous but not mandatory.

Skills

• Previous experience in a similar role is highly beneficial.
• Ability to work with patients who have complex medical and mobility challenges.
