Family Nurse Practitioner - Community Outreach
Community Health Partnership of Illinois (CHP)
Community Health Partnership of Illinois (CHP) is looking for a compassionate Family Nurse Practitioner to support primary care in the Princeville and Mendota area. You will provide high-quality, evidence-based care across a diverse patient population. Your responsibilities include conducting physical exams, diagnosing conditions, and collaborating with healthcare providers to ensure comprehensive care. Qualified candidates should possess a master's degree, RN licensure, and FNP certification. Experience in primary care is preferred.
