Home Infusion Clinical Pharmacist - Los Angeles, California

A growing infusion pharmacy in Los Angeles, CA is seeking two skilled Clinical Infusion Pharmacists for direct hire positions. This full-time, on-site role offers the opportunity to apply specialized knowledge in infusion and specialty medications while delivering comprehensive clinical and dispensing services. Schedule/Hours: Monday to Friday, 9:00 AM to 5:00 PM Responsibilities: Review and verify prescriptions for infusion and specialty medications. Perform clinical assessments ensuring appropriate medication selection, dosing, and monitoring based on evidence-based guidelines. Prepare and check compounded sterile products in compliance with USP and regulatory standards. Collaborate with prescribers, nurses, and healthcare teams to develop and manage therapy plans. Counsel patients on medication use, side effects, and adherence. Coordinate treatment logistics and manage medication inventories. Document clinical interventions and support quality assurance, safety, and compliance initiatives. Serve as a clinical resource to patients, caregivers, and healthcare professionals. Contribute to continuous improvement efforts within the pharmacy. Qualifications and Desired Experience: Doctor of Pharmacy (PharmD) or equivalent degree with an active, unrestricted pharmacist license in California. Proficiency in sterile and non-sterile compounding according to USP and state/federal standards. Experience in infusion therapy, specialty pharmacy, or hospital/clinical pharmacy settings preferred. Knowledge of insurance prior authorization, specialty medication reimbursement, and patient assistance programs is a plus. Strong clinical judgment, keen attention to detail, and dedication to patient safety and quality care. Excellent interpersonal and communication skills to effectively collaborate with multidisciplinary teams. Infusion specialty focus in neurology and oncology preferred.
