Work Environment\n

Your work will be grounded in our Hospice Center, where you will provide essential services to clients with diverse needs.

Key Responsibilities\n

• Conduct comprehensive assessments to identify and analyze clients' social, emotional, and environmental challenges, ensuring tailored responses to their unique situations.

• Develop personalized service plans that encompass specific goals, interventions, and outcomes, monitoring progress through regular follow-ups and adjustments.

• Provide counseling and support to individuals and families facing various life challenges, employing evidence-based practices to enhance their coping strategies and resilience.

• Facilitate crisis interventions when necessary, including risk assessments and safety planning, to ensure the well-being of clients experiencing acute distress.

• Engage collaboratively with clients and their families to help connect them with community resources and services needed to improve their circumstances.

• Work closely with community agencies and organizations to coordinate comprehensive services and effectively advocate for clients' rights and needs.

• Participate actively in community outreach initiatives to raise awareness about our services, empower the public, and build meaningful connections within the community.

• Contribute to team efforts by attending case conferences, participating in regular team meetings, and participating in staff training and development activities.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
